In order to start the quote for your business, along with the information you will supply in the form, we will need three different documents to be able to fully process your quote.  See the list below for the items that are remaining.

Documents Needed:

  • the last L&I/Workers’ Compensation Report
  • the last State Tax Quarterly Report/OQ Report
  • copy of the detailed payroll report from the last pay period
